Patient Declaration
Before an individual takes a genetic test, it is important that they understand the testing procedure, the benefits and limitations of the tests as well as possible consequences of the test results. The process of educating the individual regarding all implications of the test and obtaining permission is called informed consent.
By consenting to the carrying out of the genetic investigations indicated on the test request form, I understand that:
-
I am voluntarily providing a saliva sample for DNA extraction and testing, which will not be used for any other test, and will be stored for the purpose of re-examination of results and discarded accordingly once all examinations are completed.
-
I agree to the recording of my personal data, including my name, date of birth and my relationship to other family members and of required medical data, in accordance with data protection regulations.
-
I further agree that the results of the tests may be recorded for purposes of their transmittance to me or my attending physician in accordance with data protection regulations. All results from my test will be kept confidential and will not be released to any unauthorised third party.
-
I have been comprehensively informed by my physician about the disease/s to be analysed and the diagnostic differentiation between similar diseases, and their genetic basis and currently have no further questions.
-
It has been pointed out that the test I have requested is only applicable to the genes checked above or to their differentiation and that no conclusion can be drawn regarding other diseases or potential dispositions. I have also been informed about the significance and consequences of the tests. I have been informed that, especially in the case of familial genetic investigations, correct information about the relationships between family members is of extreme importance. Should I have further questions I can contact my attending physician.
-
I have been informed regarding the fact that subsequent to providing consent to genetic analysis and medical consultation, I am entitled to refuse to learn about the results of the analysis and the deriving consequences at any time. A cancellation fee applies to all cancellations once samples have been submitted to GeneCare.
-
These tests are designed to only detect certain variants of genes. The tests are not designed to detect other variants of the genes tested for.
-
In cases where more than one member of a family are tested, non-paternity may be indicated.
-
There are many environmental factors and other genetic information which contribute to a person’s susceptibility to develop certain genetic disorders. I understand that carrying a “high-risk” genotype for a condition does not imply that I will certainly develop that condition: moreover that I am at risk of developing the condition. And not presenting a “risk” genotype does not mean that I will not develop the condition, due to other contributing factors.
